Please do the following three steps to register a change of name:
1. Please download the the form for change of name here
2. Please enclose a simple letter containing the following information:
- Present adress
- Personal number (see personal number section if you do not know what this is, an application without a personal number will not be processed).
- The full old name
- The full new name (please underline the last/family name)
3. The form with the enclosed letter is then sent to:
